Carolyn W. Quist, DO, PA and Revitalize Wellness Center, PA are closely following the most up-to-date announcements and information on the known cases of Novel Coronavirus (COVID-19). Because this information is always changing, we will be monitoring all updates from the World Health Organization and Centers for Disease Control.
Face Masks Now Required for Visits
The state of Texas now requires all patients to wear a mask to enter the office. Due to the shortage of masks we ask you to bring your own, if you can’t get one, we will provide one for you as long as we have supplies. If we run out of supplies, we may have to reschedule the appointment. Sorry for the inconvenience but we want to do our part and not spreading the virus. Thanks, Dr. Quist
Routine visits are allowed and we can see all problems as well. We are maintaining the strictest of cleanliness and are fogging for viruses after hours to doubly clean the office. So be rest assured you are safe to come in and be taken care of for all your needs. We are maintaining the social distancing rules so once you have parked at the office please call (817) 926-1313 before you come in to make sure there is not a back up of checked in patients in the waiting room. If you have traveled back in state recently please stick to the quarantine schedule that the Governor has mandated and call the office with problems.
- If you are experiencing cold or flu symptoms, please make sure to contact us via phone prior to your appointment. You may also contact us for any additional questions by calling our office at (817) 926-1313.
- We are asking only the PATIENT enter our office
- We are asking all to use hand sanitizer upon arrival and we appreciate our patient’s understanding and patience as we do our part not to spread this virus.
- If someone in your household has tested positive for COVID-19, the household is ordered to isolate at home. Members of the household cannot go to work, school, or any other community function until cleared by a medical professional but may seek medical services as needed from medical personnel and facilities that are equipped to deal with the virus.
